CBI Home Health in Hamilton, Ontario is seeking an Occupational Therapist to join our Integrated Care model, delivering hospital-to-home support. You will assess and implement OT interventions, recommend equipment, and coordinate with hospital and community teams to help clients recover and regain independence.

This role emphasizes client-centered care, home modifications, and health coaching, with digital health tools for documentation and care delivery.

Qualifications

• Registered member in good standing with the College of Occupational Therapists of Ontario.

Salary Range: The salary range for this position is $82.80-$95.22
