Salary Range: The salary range for this position is $57.44 – $67.39 per visit.

• Assessment & Implementation: Provide OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
• Team Collaboration: Use a collaborative, interdisciplinary approach to care and services.
• Holistic Care: Support a restorative approach to health, offering interventions, health coaching, and self-management support.
• Digital Health: Utilize our digital platforms for care delivery, collaboration, documentation, and intervention.

• Mobility: Ability to travel within the community - valid driver’s license and insurance required.
