Job summary

Yee Hong Centre for Geriatric Care seeks a Home Support Worker for permanent night shifts, providing in-home support and emergency response within the Greater Toronto Area. Responsibilities include following care plans, documenting conditions, and maintaining client records with a client-centred approach.

Qualifications

• Graduate of an approved Personal Support Worker course in Ontario or equivalent.
• Current CPR & First Aid Certificate.
• Registration with Health and Supportive Care Providers Oversight Authority (HSCPOA) preferred.
• Client-centred care focus with strong customer service and interpersonal skills.
• Demonstrated ability to take initiative, problem-solve and time manage.
• Ability to communicate in English and Chinese.

Responsibilities

• Applies client-centred approach and respects client wishes, rights, dignity and autonomy.
• Provides high-quality in-home support per care plan and regulatory standards.
• Responds promptly to client emergencies and takes appropriate action.
• Documents client condition verbally and in reports.
• Maintains up-to-date progress notes and statistics of service.
• Collaborates with caregiver to provide optimal care and emotional support.
