Sentrex Health Solutions is seeking a Supervisor, Field Nurse Case Managers / Nurse Care Navigators to lead a field-based clinical team delivering patient support programs across clinics and health care providers in Canada.

The role focuses on ensuring consistent service delivery, timely education, enrolment, and adherence support. Strong leadership, clinical expertise, and cross-functional collaboration are essential for success.

Qualificações

• Must be a Registered Nurse in good standing with the respective provincial college.
• 3+ years of clinical experience, preferably in immunology or related area.
• Experience supporting field-based teams is an asset.
• Bilingual English and French is an asset.

Responsabilidades

• Provide day-to-day supervision and coaching to Field Nurse Case Managers and Nurse Care Navigators.
• Conduct regular clinic visits to reinforce program objectives and strengthen relationships with health care providers.
• Support in-clinic and remote patient education, enrolment support, and adherence activities.
• Escalate clinical and operational issues following program protocols and policies.
