Job summary

Sentrex Health Solutions Inc. is seeking a Field Nurse Case Manager to coordinate patient support services across Atlantic Canada.

This field role involves travel to clinics, enrolling patients, and providing education on diseases and therapies while liaising with physicians and health care providers. You will work within a patient services team, support program delivery, and ensure timely access to therapies.

Qualifications

• Must be a Registered Nurse in good standing.
• 2-4 years of clinical experience.
• Experience in Patient Support Program is an asset.
• Experience in oncology, gastro or rheumatology.
• Bilingual in English and French is an asset.

Responsibilities

• Coordinate and deliver patient support services for programs.
• Register HCP and patients per program protocols.
• Assist with enrolment and funding coordination.
• Travel to physician offices and clinics as needed.
• Provide disease and therapy education to patients.
• Maintain confidentiality and comply with pharmacovigilance.
