Job summary

Sentrex Health Solutions is seeking a Field Nurse Case Manager to join the Patient Services team. This field-based role coordinates and delivers patient support programs, working across clinics within the assigned geographic region and assisting healthcare providers with enrolment.

The position is hybrid in Ontario, with travel up to 75% and regular training. You must be a Registered Nurse with at least 2 years of clinical experience; bilingual in English and French is an asset.

Qualifications

• Must be a Registered Nurse in good standing with the College of Nurses of their respective province.
• 2+ years of clinical experience.
• Bilingual in English and French is an asset.

Responsibilities

• Coordinate and deliver patient support services across clinics within the assigned geographic region.
• Assist physicians with patient enrolment and information collection for forms.
• Provide education and disease/product information to patients and healthcare providers.
• Support funding and reimbursement processes for therapies and patient care.
