Sentrex Health Solutions is seeking a Field Nurse Case Manager to coordinate patient support programs across Montreal and Ottawa. You will travel to clinics, enroll patients, and support physicians with accurate documentation and timely access to therapies.

The role requires an actively licensed RN with 2–4 years of clinical experience; oncology/gastro/rheumatology experience is a plus, and bilingual ability is an asset. A competitive salary and comprehensive benefits accompany this position.

Qualifikationen

• Must be a Registered Nurse in good standing with the College of Nurses of their respective province.
• 2-4 years of clinical experience.
• Experience in Patient Support Program is an strong asset.
• Experience in oncology, gastro or rheumatology.

Aufgaben

• Adhere to Sentrex Policies and Procedures.
• Perform HCP and patient registration activities, according to program protocols.
• Process data entry activities of pertinent information.
• Provide answers to general inquiry calls about program services.
• Provide medical support to patients and healthcare providers, including education on diseases and therapies.
• Attend extensive training sessions for therapies provided by the pharmaceutical team.
• Telephone support inbound and outbound as needed.
• Manage case-management protocols including clinical monitoring and follow-up.
• Assist with funding searches for patient therapies.
• Attend patient meetings and conferences to educate on disease/product information.
• Identify and report adverse events to Health Canada and related systems.
• Maintain confidentiality of client and corporate information.
• Liaise with physicians’ offices and other providers for coordinated service.
