Job summary

Sentrex Health Solutions is hiring a Nurse Case Manager to coordinate patient support programs and liaise with health care providers to ensure access to prescribed therapies. Reporting to the program manager, you will perform registration, data entry, patient education, and reimbursement coordination, with flexibility to support patients and providers across Canada.

Qualifications

• Must be an RN or RPN in good standing with the College of Nurses of their province.
• Experience in Patient Support Programs is required.
• Minimum 2 years of clinical experience (preferred).

Responsibilities

• Perform HCP and patient registration activities, according to program protocols and applicable departmental procedures.
• Process data entry activities of pertinent information received at different points of contact during the delivery of program services.
• Provide medical support to patients and health care providers, including education on diseases and therapies and disease management.
• Prepare for and attend specialized training sessions for therapies provided by the pharmaceutical team.
• Telephone support and adherence follow-up as needed.
• Adhere to case-management protocols, including clinical monitoring and adverse event reporting.
• Work with the reimbursement specialist and physician to secure funding for therapies.
• Attend patient meetings to educate patients and providers about disease/product information.
• Develop training material and programs for PSP team members.
• Perform patient enrolment into the program and complete required follow-ups.
• Provide ongoing support on disease state or product related questions to ensure patient adherence.
• Follow program protocols to ensure continuity of service delivery to patients.
• Report and document Adverse Events per pharmacovigilance training.
