Job summary

Sentrex Health Solutions Inc. is seeking a Bilingual Nurse Case Manager to support patient programs remotely across Canada. You will coordinate services, liaise with HCPs, and assist with coverage and reimbursement for prescribed therapies.

You must be an RN or RPN in good standing with your provincial College, with at least 2 years of clinical experience and exposure to patient support programs. Neurology experience is a plus and a flexible 8am-8pm EST schedule is available.

Qualifications

• Must be an RN or RPN in good standing with the College of Nurses in their province.
• Experience in Patient Support Programs is required.
• Minimum 2 years of clinical experience (preferred).
• Reimbursement knowledge to obtain coverage for patients is an asset.
• Experience in Neurology is a strong asset.

Responsibilities

• Register HCP and patient according to program protocols.
• Enter pertinent information at various contact points during service delivery.
• Provide medical education on diseases/therapies and disease management.
• Coordinate funding searches and work with reimbursement specialists.
• Liaise with physicians, pharmacies and other providers to coordinate care.
