Qualifications

• Bachelor's degree or better in Human Services or related field.

Responsibilities

• Assist clients in developing and maintaining positive working relationships with professionals to support wellness.
• Assist and encourage clients in pursuing leisure activities, exercise programs and community socialization.
• Support clients in self-management, coping strategies, confidence and community rehabilitation.
• Guide clients toward daily living goals and eventual housing stability.
• Provide information, referrals, and advocacy for clients and navigate systems processes.
• Accompany clients to appointments to access community resources and housing options.
• Maintain professional relationships with community contacts and educate partners about referral processes.
