Qualifications

• Bachelor's degree or better in Human Services or related field.

Responsibilities

• Life Skills Support: Assist clients in developing and maintaining relationships with professionals to support wellness.
• Life Skills Support: Encourage and assist clients in pursuing leisure activities, exercise, and socialization.
• Life Skills Support: Help clients develop personal coping strategies, self-confidence, and community reintegration.
• Assist clients in achieving daily living goals to progress toward independent living and housing.
• Provide guidance and referrals on personal finance and legal issues; help navigate systems.
• Support clients to become active community members and role models.
• Program Support: Develop housing stability-focused plans and maintain relationships with housing authorities and landlords.
• Navigate community services, coordinate resources, and attend court when needed.
• Advocate for clients and accompany them to appointments for resources and support.
• Complete documentation, reports, and statistics per guidelines.
