Job summary

AgeCare is seeking a Registered Practical Nurse (RPN) to provide professional nursing care to residents in a long-term care setting in Ontario. You will work under the guidance of a Registered Nurse and support Personal Support Workers to ensure high-quality care.

Responsibilities include collaborating with physicians, coordinating care with the health team, and participating in shift reports to keep the team informed of resident changes. Ontario certification and active practice are required.

Qualifications

• Geriatric, rehabilitation or long-term care nursing knowledge.
• Familiar with relevant health acts and standards.
• Active registration with College of Nurses of Ontario.

Responsibilities

• Collaborate with the RN to implement AgeCare policies and procedures on the unit.
• Liaise with physicians and health care team members; notify physician of significant changes if RN absent.
• Work with care team to meet residents' care needs.
• Provide direction to Personal Support Workers on the care team.
• Participate in shift handover to communicate resident status updates.
