Qualifications

• Geriatric, rehabilitation or long-term care nursing background.

Responsibilities

• Coordinate nursing and services for residents within the neighbourhood and residence.
• Lead shift report with health care team members and communicate changes in resident status.
• Document significant changes in residents’ condition in line with policies and standards.
• Process physician orders promptly and escalate processing issues to leadership.
