AgeCare is seeking a Registered Practical Nurse to provide professional nursing care to residents in long-term care facilities and collaborate with the health care team on the unit. The role includes directing Personal Support Workers and contributing to shift reports to ensure continuity of care.

The ideal candidate holds a Certificate of Competence from the College of Nurses of Ontario and is in good standing, with knowledge of applicable statutes and best practices in geriatric care.

Qualifications

• Knowledge of geriatric, rehabilitation or long-term care nursing.
• Knowledge of Canadian health acts and nursing standards.
• Current Certificate of Competence from College of Nurses of Ontario.

Responsibilities

• In collaboration with the Registered Nurse, implements AgeCare policies and procedures on the unit.
• Provides input into procedure revisions within the residence and the unit.
• Liaises with physicians and other members of the health care team including external health care partners.
• In the absence of the Registered Nurse, notifies the physician of significant changes in resident status.
• Works with health care team to ensure residents’ care needs are met.
• Provides direction to Personal Support Workers on the care team.
• Participates in shift reports to inform the team of changes in resident status.
