Job summary

Simplicant, Inc. is seeking a Personal Support Worker to provide resident-centered care in our Ontario long-term care setting.

You will assist with ADLs, support mobility, document care in PointClickCare, and follow the resident’s plan of care with compassion and respect. Qualifications include a PSW certificate (600+ hours), registration with the appropriate Ontario body preferred, strong interpersonal skills, and commitment to IPAC and safety protocols.

Qualifications

• PSW Certification with at least 600 hours, including practical experience.
• Ontario postsecondary institution or district school board issuing PSW certificate.
• Experience with person-centered care, strong interpersonal skills.
• Ability to document accurately in electronic records.

Responsibilities

• Promote the Vision, Mission and Values of the Home.
• Assist with personal care and ADLs according to the plan of care.
• Encourage independence and dignity while supporting mobility and feeding.
• Support bathing routines, respecting preferences and safety.
• Collaborate with Nursing/Restorative team to meet restorative goals.
• Follow care interventions and communicate triggers to reduce escalation.
• Maintain least-restraint principles and report safety concerns promptly.
• Document shifts accurately in the electronic record (ADLs, mobility, etc.).
• Follow IPAC practices and participate in outbreak measures.
