Job summary

CBI Home Health in Peterborough, Ontario, is seeking a Physiotherapist to support clients in the community. You will deliver hands-on therapy to help patients regain strength and independence in their homes, within the Integrated Care Program.

The role focuses on reducing readmissions, improving outcomes, and providing patient-centered care with flexible scheduling and supportive resources.

Qualifications

• Registered in good standing with the College of Physiotherapists of Ontario.
• Ability to travel from home to home within the community — a valid driver’s license, vehicle and car insurance is required.
• Commitment to person and family-centered care.
• Passion, empathy and critical thinking in client decision-making.
• Strong communication and collaboration within an interdisciplinary team while working independently.

Responsibilities

• Assist patients to rebuild strength and function in their homes and communities.
• Provide hands-on therapy as part of the Integrated Care Program to reduce readmissions.
• Collaborate with other health professionals to ensure comprehensive care.
