Job summary

CBI Home Health in Peterborough, ON is hiring an Occupational Therapist to support clients at home within our Integrated Care Program. The role focuses on functional assessments, personalized interventions, and collaboration with care teams to promote safe transitions from hospital to home.

The successful candidate will be registered with the College of Occupational Therapists of Ontario, able to travel in the community, and dedicated to person- and family-centered care with strong communication

Qualifications

• Registered member in good standing with the College of Occupational Therapists of Ontario.
• Ability to travel within the community - valid driver’s license and insurance required.
• Client-focused care with empathy and strong communication.
• Excellent teamwork and independent work capabilities.
• Advocate for enabling clients to remain at home when aligned with goals.

Responsibilities

• Assess and implement OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
• Collaborate with an interdisciplinary care team.
• Provide health coaching and self-management support.
• Utilize digital platforms for care delivery, documentation, and collaboration.
