Job summary

The Care Company is seeking a passionate Physiotherapist to join its community rehabilitation team in the Toronto area. The role offers flexibility as either an employee or independent contractor, with autonomy to manage your own community caseload while receiving team support.

You will assess and treat clients in home settings, develop individualized plans, and contribute to program development and quality improvements within our growing physiotherapy services.

Qualifications

• Registered with the College of Physiotherapists of Ontario (CPO) and in good standing.
• Experience providing home and community care, preferred with HCCSS or Ontario Health atHome.
• Ability to work independently in the community with strong documentation.

Responsibilities

• Complete comprehensive in-home physiotherapy assessments within referral timeframes.
• Develop individualized, evidence-based treatment plans to maximize mobility and safety.
• Provide follow-up visits focused on therapeutic exercise, gait training and falls prevention.
• Develop home exercise programs and educate clients and caregivers.
• Recommend mobility aids and collaborate with a multidisciplinary team.
• Maintain timely, accurate clinical documentation and communicate caseload updates.
