CBI Home Health is looking for a Physiotherapist to support patients in the Bradford region with home-based care as part of our Integrated Care Program. The role focuses on helping clients rebuild strength and function after hospitalization through hands-on, community-based care.

As a PT in our team, you’ll contribute to reducing readmissions and improving outcomes while maintaining client-centered goals. Flexible scheduling and comprehensive benefits are highlighted in our total rewards.

Qualifications

• Registered in good standing with the College of Physiotherapists of Ontario.
• Ability to travel from home to home within the community with a valid driver’s license, vehicle and car insurance.
• Commitment to person- and family-centered care.
• Strong communication and collaboration within an interdisciplinary team.

Responsibilities

What you will need to be successful

• Registered in good standing with the College of Physiotherapists of Ontario
