Job summary

Service Coordination for Persons With Special Needs is seeking a Case Manager for Children’s Services to coordinate supports for eligible children with developmental disabilities or autism. You will work directly with families to access community resources and implement person-centered plans, while maintaining case files and collaborating with partners.

The role requires 3+ years in developmental services, a BSW/DSW/SSW diploma, and strong communication skills.

Qualifications

• Minimum of 3 years experience in developmental services.
• Diploma in BSW/DSW/SSW required.
• Proficient in MS Office and data management.

Responsibilities

• Maintain accurate case files and agency database.
• Develop person-centered plans with individuals.
• Provide information and referrals to access community services.
• Attend meetings and coordinate transitions with schools when applicable.
• Support budgeting and financial forecasting where applicable.
• Promote awareness of SCS and participate in community outreach.
