The role requires a Bachelor's degree in a related field and at least 1 year of social services experience, with ASD knowledge and strong communication skills.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related human services field.
• Minimum one year of experience in social work, case management, or human services.
• Knowledge of Autism Spectrum Disorder (ASD) required.
• Knowledge about health insurance, local human service resources, family supports, and care coordination.
• Excellent interpersonal, communication, and relationship-building skills.
• Proficiency with Microsoft Office and electronic medical record/documentation systems.
• Ability to meet driver qualifications to drive personal vehicle for program needs.

Responsibilities

• Guide families through the autism screening and assessment process from initial contact through service coordination.
• Facilitate autism screenings and provide education and support to families regarding screening outcomes and next steps.
• Serve as the primary point of contact for assigned families, providing ongoing case management, advocacy, and resource navigation.
• Develop individualized service plans and coordinate referrals to community resources and Easterseals programs.
• Collaborate with physicians, therapists, community partners, and multidisciplinary team members to ensure quality, person-centered care.
• Educate families on available services, intervention options, benefits, and community supports.
• Participate in community outreach activities, educational events, and autism awareness initiatives.
• Assist in developing educational resources, workshops, and materials for families and community partners.
• Maintain accurate client records, service documentation, and program data while ensuring compliance with reporting requirements and timelines.
• Support grant reporting, quality assurance activities, and program evaluation through data tracking, record review, and administrative support for Autism Services leadership.
