Job summary

Your Support Services Network (YSSN) in Aurora, Ontario seeks a Developmental Services Case Manager to deliver intensive, person-centered services for children and adults with developmental disabilities and dual diagnosis. The role supports navigating services, accessing resources, and improving quality of life.

The position is a full-time contract to August 2027 with in-office requirements, travel within York Region, and a commitment to confidentiality and collaboration in a diverse workplace.

Qualifications

• Bachelor’s degree in social work or related discipline.
• Minimum three years directly related front line experience.
• Experience with individuals who have developmental disabilities is essential.

Responsibilities

• Provide intensive case management, advocacy, and support services.
• Conduct needs assessments and develop individualized service plans.
• Facilitate referrals and support skill development for independence and community participation.
