Qualifications

• Minimum 2 years' experience helping individuals with complex needs (disabilities, mental health, dementia).
• Compassionate, patient approach with a genuine desire to help clients.
• Strong communication and interpersonal skills; build trusting relationships with clients and families; documentation.

Responsibilities

• Assist clients with daily living activities including personal care, meals and household tasks.
• Provide individualized support and implement care plans.
• Engage clients in activities and outings to promote wellbeing and independence.
• Monitor progress and report changes to the care team.
• Maintain accurate client records and documentation.
• Collaborate with the care team to ensure a person-centred approach.
