Job summary

Wallara Australia is seeking a part-time Support Coordinator to join our Client Experience team at our Keysborough head office. You will empower clients to activate their NDIS plans and manage coordination hours, while building strong relationships with families, clients and staff.

You'll thrive in a busy environment, bringing at least 2 years in a support coordination role and experience with individualised plans.

Qualifications

  • Minimum 2 years' experience in a Support Coordination role
  • Demonstrated experience delivering on individualised plans
  • Experience supporting people with disabilities and knowledge of community services
  • Knowledge and understanding of the National Disability Insurance Scheme (NDIS)
  • Excellent written and verbal communication skills, with the ability to interact at all levels and produce concise notes and reports
  • Exceptional problem solving and analytical skills to identify issues and options for positive client outcomes
  • Client-focused with ability to work with diverse people to deliver high quality outcomes
  • Commitment to WHS, EEO and cultural diversity

Responsibilities

Job description

We currently have a fantastic opportunity in our Client Experience team for a Support Coordinator. It is a part-time position, located at our head office in Keysborough. You will provide support coordination services to our clients, helping them achieve positive outcomes.

You’ll need to thrive in a busy, fast paced environment and have fantastic interpersonal skills.

Work pattern: 3 days per week, 8:30am – 4:30pm or 9am - 5pm. Days are flexible but Wednesday availability is essential. Flexibility is required to add an extra day when needed.

Key Responsibilities
  • Empowering clients to activate their NDIS plans and ensuring outcomes are achieved within their allocated support coordination hours
  • Respond to potential participant enquiries in a timely manner
  • Build and maintain positive relationships with stakeholders, including families, clients, work teams, and the wider community
  • Attend NDIS pre‑planning meetings (as required) and NDIS planning meetings with clients
  • Support the transition of new clients and their families into services
  • Maintain and update Support Coordination logs
  • Process NDIS billing accurately and in a timely manner
  • Prepare and complete NDS documentation and reporting as required.
What you'll need to succeed
  • Minimum 2 years' experience in a Support Coordination role
  • Demonstrated experience in delivering on individualised plans
  • Previous experience supporting people with disabilities and knowledge of community services
  • Knowledge and understanding of the National Disability Insurance Scheme (NDIS)
  • Excellent written and verbal communication skills, with the ability to interact effectively at all levels within the organisation and produce factual, concise file notes and reports
  • Exceptional problem solving, analytical and evaluation skills to enable the identification of issues, analysis of options and the judgement to determine appropriate courses of action relevant to achieving positive outcomes for clients
  • A client first outlook with well-developed customer relations skills and the ability to work with a range of people to deliver high quality outcomes
  • Excellent time management skills and the ability to think laterally
  • Ability to work as part of a team and independently
  • Commitment to exceptional service delivery and a demonstrated professional approach to work
  • Commitment to the principles of WHS, EEO, Cultural Diversity and all relevant legislation
  • Relevant tertiary qualification in Disability Support, Community Services, Individual Support or equivalent qualification.
