Job summary

Wallara Australia is seeking a part-time Support Coordinator to join our Client Experience team at our Keysborough head office. You will empower clients to activate their NDIS plans and manage coordination hours, while building strong relationships with families, clients and staff.

You'll thrive in a busy environment, bringing at least 2 years in a support coordination role and experience with individualised plans.

Qualifications

• Minimum 2 years' experience in a Support Coordination role
• Demonstrated experience delivering on individualised plans
• Experience supporting people with disabilities and knowledge of community services
• Knowledge and understanding of the National Disability Insurance Scheme (NDIS)
• Excellent written and verbal communication skills, with the ability to interact at all levels and produce concise notes and reports
• Exceptional problem solving and analytical skills to identify issues and options for positive client outcomes
• Client-focused with ability to work with diverse people to deliver high quality outcomes
• Commitment to WHS, EEO and cultural diversity

Responsibilities

• Empower clients to activate their NDIS plans and manage their support coordination hours
• Respond to participant enquiries promptly
• Build and maintain relationships with families, clients, work teams and the community
• Attend NDIS meetings with clients
• Support transition of new clients into services
• Maintain and update Support Coordination logs
• Process NDIS billing accurately and timely
• Prepare and complete NDS documentation and reporting
