Job summary

Wellways Australia is seeking a part-time Support Coordinator to assist people living with a disability in Ballarat, VIC. The role focuses on capacity-building, helping participants design and manage their supports, and enabling choice and control on their recovery journey.

The position offers pro rata salary, with a proactive, inclusive culture and opportunities for ongoing training and development.

Qualifications

• Experience working with people living with a disability or mental health
• Ability to participate in out-of-hours activities as required
• Right to work in Australia with a valid driver's license
• Completion of NDIS Worker Screening Check and related checks
• NDIS Worker Orientation Module Certificate

Responsibilities

• Delivery of NDIS funded Support Coordination.
• Provide capacity-building support to participants to design, manage and control their supports.
• Help participants develop relationships, build skills and establish support networks.
• Collaborate with participants to ensure safe, inclusive and empowering outcomes.

The Support Coordinator is responsible for the delivery of NDIS funded Support Coordination.
