Job summary

Marymead CatholicCare Canberra & Goulburn – O'Connor ACT is seeking a Seniors in Networked Communities (SINC) Administrator to coordinate CHSP aged care services across the ACT and Southern Highlands. You will support referrals, onboarding, rostering, and client records in a person-centred, wellness-focused approach aligned with My Aged Care plans.

You will work with internal teams and external partners to ensure timely, appropriate services, maintain compliance with CHSP requirements and

Qualifications

• Certificate in Aged Care or similar.
• Strong computer literacy, including Microsoft Office and client management systems.
• Excellent communication and interpersonal skills.
• High level of attention to detail and ability to learn new systems quickly.
• Strong organisational and time management skills; multitask and meet deadlines.

Responsibilities

• Managing referrals and maintaining the client waitlist.
• Coordinating the onboarding of new clients to the program.
• Liaising with rostering to support timely delivery of services.
• Undertaking rostering responsibilities as required.
• Liaising with clients to understand needs and ensure services align with My Aged Care plans.
• Monitoring changes in client needs and refer back to My Aged Care as needed.
• Maintaining accurate client records and documentation.
• Collaborating with internal teams and external stakeholders for high-quality delivery.
• Ensuring CHSP requirements and quality standards are met.
• Participating in On Call as per policy.
