Job summary

Carinity is seeking a Community CHSP Care Partner to support seniors living safely at home across multiple regions. You will coordinate the client journey from admission through reviews and discharge, and help develop and monitor individual care plans within approved services.

As a warm, organised professional, you will liaise with families, providers and staff, maintain documentation quality, and ensure compliance with the Strengthened Aged Care Quality Standards and related frameworks. 18+.

Qualifications

• Certificate III in Aged Care or related program completed.
• Experience with Consumer Directed Care or care management models.
• Knowledge of Strengthened Aged Care Quality Standards, SIRS and Aged Care Act 2025.
• Current NDIS Worker Screening and a valid driver’s licence.

Responsibilities

• Coordinating each client’s journey with Carinity from admission through reviews and discharge.
• Support clients to develop, assess and maintain budgets and care plans aligned to services.
• Ensure care services reflect a person-centred, strengths-based approach.
• Monitor client wellbeing, referrals, assessments and care reviews.
• Liaise with Lifestyle Carers, contractors and clients for scheduling and onboarding.
• Build strong relationships with clients, families, assessment teams and health professionals.
• Advocate for clients and follow up on concerns or changes in conditions.
