Job summary

Comlink Australia seeks a Care Advisor to enhance client experience by coordinating CHSP services and promoting health, wellbeing and continuity of care so clients can age in place. You’ll conduct assessments, develop care plans, monitor wellbeing and work with a multidisciplinary team to deliver person-centered care and smooth service delivery.

This role requires compassion, initiative and the ability to navigate changing client needs while maintaining high-quality documentation and reporting.

Qualifications

• Cert IV in Community Services or related field and minimum industry experience.
• Understanding of aged care act and quality standards.
• Experience in case management and home/community care preferred.

Responsibilities

• Conduct participant assessments and reviews with participants and families per CHSP guidelines.
• Develop and monitor participant-focused care plans and assist with implementation.
• Maintain high-quality assessment and care planning documentation.
• Foster collaborative, person-centered relationships to reflect needs and ACQS requirements.
• Provide comprehensive case management and monitor wellbeing to plan for longer-term service needs.
• Collaborate with team, community organisations and allied health providers for optimal outcomes.
• Record feedback and complaints and address issues per policy, reporting regularly.
