Hunter New England Local Health District is seeking a Registered Nurse for the Transitional Aged Care Program (TACP) at Wallsend Health Campus. The role supports older adults through short-term rehabilitation and care coordination, helping regain independence and ensuring a safe transition home.

You will work with a multidisciplinary team to deliver compassionate, person-centred care, aligned with NSW Health CORE values and a strong focus on culturally respectful practice.

Qualifications

• Current registration as a Registered Nurse with AHPRA.
• Current NSW Driver Licence and ability to travel within the community.
• Postgraduate qualifications or ongoing professional development in aged care, rehabilitation or community health are highly regarded.

Responsibilities

• Deliver safe, evidence-based nursing care tailored to rehabilitation goals.
• Assess clients, monitor health and respond to changes in clinical condition.
• Support older people and families to navigate healthcare services.
• Collaborate with the multidisciplinary team to coordinate care and safe transitions home.
• Advocate for clients while maintaining professional, ethical and culturally respectful nursing practice.
• Contribute to culturally safe care for Aboriginal and Torres Strait Islander peoples.
