Job summary

War Memorial Hospital in Waverley, part of Uniting NSW & ACT, is seeking a Registered Nurse to join the Transitional Aged Care (TACP) team. You will provide nursing and case management to older clients in a goal‑oriented, time‑limited program within SESLHD guidelines.

The role is a fixed-term full-time position, Monday to Friday, 8:00am‑4:30pm (or 8:30am‑5:00pm), commencing 11 January 2027 for 12 months with potential for extension. Applications close 15 November 2026.

Qualifications

• Current registration as a Registered Nurse with the Nursing & Midwifery Board of Australia (AHPRA).
• Minimum 3 years’ experience as a Registered Nurse in aged care or rehabilitation.
• Demonstrated ability to perform comprehensive nurse assessments and plan care for older people.
• Ability to work autonomously within a multidisciplinary team and manage caseloads.

Responsibilities

• Provide nursing services to Transitional Aged Care Program patients.
• Coordinate nursing care and case management in collaboration with the MDT.
• Assess, plan and monitor older patients with dementia or complex needs.
• Contribute to quality improvement and adhere to NSW Health policies.
• Participate in electronic health records and relevant portals, and ensure WHS compliance.
