Job summary

South Western Sydney Local Health District is recruiting an experienced Allied Health Leader to oversee the Transitional Aged Care Program team. You will manage case coordination, workforce planning and governance, ensuring high quality, safe care for older adults transitioning from hospital to community settings.

You will collaborate with nurses, OTs, PTs, social workers and other clinicians to maximise outcomes, while aligning with NSW Health standards and policy.

Qualifications

• Postgraduate qualifications and/or management experience or equivalent work experience.
• Senior nursing/allied health clinical experience in aged care settings with leadership experience.
• Experience in coordinated assessment and case management of older persons with complex care needs.

Responsibilities

• Provide leadership, direction and performance management to the multidisciplinary team.
• Oversee clinical governance, onboarding, case management, service delivery and discharge planning.
• Manage workforce planning, rostering, budgeting and staff development.
• Ensure compliance with legislation and standards in transitional aged care services.
• Drive continuous improvement and service innovation aligned with best practice.
• Maintain effective communication with internal and external stakeholders.
• Uphold professional conduct and address unsafe practice in the workplace.
