Hunter New England Health is seeking a compassionate Registered Nurse for the Transitional Aged Care Program in the Lower Mid North Coast region. Work across Taree, Forster, or Gloucester with a 20-hour week and permanent part-time arrangements, supporting older adults to transition safely home from hospital.

You will assess, plan and coordinate care with a multidisciplinary team to restore independence and mobility, with strong professional development and incentive opportunities available.

Qualifications

• Current registration as a Registered Nurse with AHPRA.
• Strong clinical assessment and care coordination skills.
• Effective communication, organisation and time management.
• Ability to work independently and prioritise competing demands.
• A genuine passion for supporting older people to regain independence.
• Experience in community nursing, rehabilitation, aged care, discharge planning or transitional care is highly regarded.

Responsabilités

• Completing patient assessments and coordinating referrals from hospital.
• Developing care plans that support recovery, mobility and independence.
• Managing patient care and monitoring progress throughout the program.
• Working with patients, families and carers to establish meaningful goals.
• Coordinating ongoing support and safe transitions from the program.
• Collaborating with nursing, allied health and community support teams.
• Managing a varied community caseload across the Lower Mid North Coast.
