Job summary

NSW Health is seeking an Enrolled Nurse to join the Transitional Aged Care Program in a permanent full-time role in Sydney. You will deliver person-centred care for older adults aiming to regain independence and health goals.

Required registration with AHPRA (Division 2) and demonstrated community nursing skills. The role offers flexibility and opportunities to work within a supportive multidisciplinary team.

Qualifications

• Current registration as an Enrolled Nurse (Division 2) with AHPRA and no medication notations.
• Nursing knowledge in Community Nursing (skin assessments, risk identification, escalation of clinical deterioration).
• Person-centred approach to nursing with openness to new care delivery methods.
• Ability to work independently with minimal supervision while collaborating in a multidisciplinary team.
• Effective clinical communication using IT, written and verbal skills.

Responsibilities

• Deliver person-centred care to older adults in transitional aged care.
• Conduct skin assessments and identify risks, escalating deteriorations when needed.
• Work collaboratively within a multidisciplinary team to achieve high-quality outcomes.
• Communicate clearly using IT tools and written/verbal communication.
