Job summary

War Memorial Hospital in Waverley, NSW, is seeking a Registered Nurse for the Transitional Aged Care Program. The role involves delivering comprehensive nursing and case management services within a multidisciplinary TAC team, aligning with NSW Health and SESLHD standards.

The position is a fixed-term full-time role, Monday–Friday, 8:00am–4:30pm or 8:30am–5:00pm, commencing 11 January 2027 for 12 months with potential extension. Eligibility for an ongoing list may follow.

Qualifications

• Current RN registration with the Nursing & Midwifery Board of Australia and a NSW Drivers' license Class C.
• Minimum 3 years’ experience as a Registered Nurse in aged care, rehabilitation or related specialty with post-graduate qualification or pursuing same.
• Experience or interest in community-based nursing assessments, care and case management for older people, including those with dementia.
• Ability to work autonomously and within a multi-disciplinary team.
• Strong written and verbal communication, interpersonal skills across levels and departments.
• Experience in delegating and supervising nursing duties to ensure quality care and efficient resource use.
• Commitment to professional development, evidence-based practice and quality improvement.
• Familiarity with health information systems; comfortable with EMR and MS Office tools.

Responsibilities

• Plan, coordinate and deliver nursing and case management interventions for Transitional Aged Care patients.
• Participate in quality initiatives within the TAC team and hospital.
• Contribute to multidisciplinary teamwork to achieve patient outcomes within timeframes.
• Engage in professional development and compliance with NSW Health policies.
