Job summary

NSW Health is seeking a Transitional Aged Care Programme Registered Nurse at Cumberland Hospital, North Parramatta. The role supports older adults to regain independence after hospitalisation and works within a multidisciplinary team to deliver high-quality, person-centred care.

The position is temporary part-time (24 hours/week) with a NSW driving licence and opportunity to contribute to better outcomes for older people in the community.

Qualifications

• Bachelor of Nursing or higher and current AHPRA/NMRB registration.
• Experience in aged care, rehabilitation or community health settings.
• Proven ability to work in a multidisciplinary team and deliver person-centred care.

Responsibilities

• Provide holistic nursing care to optimise physical and cognitive function post-hospitalisation.
• Plan, implement and evaluate individualised care plans.
• Collaborate with a multidisciplinary team of nurses, physiotherapists, occupational therapists and social workers.
• Maintain accurate clinical documentation and uphold professional standards.
