Better Health Network is seeking two experienced Case Managers for the Pathways Program (RhED). These fixed-term roles focus on outreach-based, person-centred case management for current and former sex workers across Victoria.

You will work within RhED to help clients navigate complex services, overcome barriers, and achieve personal goals in a trauma-informed, harms-reduction framework. Based at the Victorian Pride Centre, St Kilda, with additional community work as required.

Qualifikationen

• Relevant tertiary qualifications in Social Work, Community Development, Criminology, Psychology, Counselling or a related discipline.
• Demonstrated case management and assessment experience with marginalised, disadvantaged people with complex needs.
• Experience in a trauma-informed framework and understanding of stigma, discrimination, criminalisation and systemic barriers.
• Strong advocacy, risk assessment and risk management skills, including understanding the MARAM Framework.
• Understanding that sex work is work and knowledge of the sex industry.
• Knowledge of harm minimisation/harm reduction and the Social Model of Health.
• Strong communication, organisational and time management skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Cultural awareness and ability to work with diverse communities.
• Current full or probationary driver's licence.

Aufgaben

• Provide person-centred, trauma-informed case management to current and former sex workers.
• Manage a complex caseload with assessment, planning, review, transition and closure.
• Offer information, referrals, advocacy, practical assistance and psychosocial support.
• Support clients with complex issues including health, housing, and financial hardship.
• Develop and review individualised support plans based on client goals and needs.
• Provide sex worker-informed support, safety planning and harm reduction information.
• Build referral networks and work with external services to improve outcomes.
• Maintain accurate, confidential case records and contribute to reporting.
• Participate in after-hours outreach as required, including nightlife services.
