Job summary

Better Health Network is seeking two experienced Case Managers for the Pathways Program within RhED, Victoria. You will deliver person-centred, outreach-based case management to current and former sex workers, helping them navigate services and achieve personal goals.

You will work in a trauma-informed framework, develop individualized plans, and collaborate with multidisciplinary teams while maintaining thorough records and contributing to program reporting. National pre-employment checks apply.

Qualifications

• Relevant tertiary qualifications in Social Work, Community Development, Community Services, Criminology, Psychology, Counselling or a related discipline.
• Experience in case management and assessment with marginalised people.
• Experience in trauma-informed practice and understanding of stigma, discrimination and systemic barriers.
• Strong advocacy, risk assessment and risk management skills, including knowledge of the MARAM Framework.
• Understanding that sex work is work and knowledge of the sex industry and its legislative environment.
• Knowledge of harm minimisation and the Social Model of Health.
• Strong communication, organisational and time management skills.
• Ability to work independently and collaboratively within a multidisciplinary environment.
• Well-developed cultural awareness and ability to work with diverse communities.
• A current full or probationary driver’s licence.
• Lived experience of working in the sex industry will be highly regarded.

Responsibilities

• Providing person-centred, trauma-informed and outreach-based case management to current and former sex workers.
• Managing a complex caseload with assessment, planning, review, transition and closure.
• Providing information, referrals, advocacy, practical assistance and psychosocial support.
• Supporting clients facing complex issues such as mental/physical health, substance use, homelessness, family violence and financial hardship.
• Developing and reviewing personalised support plans based on client goals and needs.
• Providing sex worker-informed support, safety planning and referrals.
• Building referral networks and coordinating with external services to improve outcomes.
• Maintaining confidential case records and contributing to reporting.
• Participating in after-hours service delivery as required (evening brothel outreach and Friday drop-in).
