Job summary

Better Health Network is seeking a passionate Case Manager - Complex Care to join our Pre-Housing Homelessness Program. Based at the Victorian Pride Centre, St Kilda, you’ll walk alongside people experiencing homelessness as they navigate health, housing and community supports toward stability and housing outcomes.

You’ll work as part of a multidisciplinary team in a health-led model, using outreach, practical support and advocacy to help clients access services, maintain safety and move toward

Qualifications

• Relevant tertiary qualifications in Social Work, Psychology, Community Services or a related field.
• Demonstrated case management experience with homeless and marginalised adults.
• Strong assessment, planning and advocacy skills.
• Ability to conduct outreach and build rapport in diverse settings.
• Knowledge of health and community services impacts on wellbeing.
• Clear written and verbal communication, organisation and documentation.
• Ability to respond effectively in crisis situations.
• Ability to build productive relationships with health, housing and community partners.

Responsibilities

• Help people experiencing homelessness access healthcare, housing, mental health, AOD, legal aid and community connections.
• Provide assertive outreach and practical support to promote health, independence and stability.
• Work within a multidisciplinary team to deliver a health-led model of care.
