Job summary

Focused on Care's Kempsey office is seeking an experienced NDIS Support Coordinator to join our growing team. The role is office-based with travel to the surrounding region to support clients.

You will implement NDIS plans, coordinate services, and build relationships with providers. You must have experience in support coordination, excellent communication, and a passion for person-centred care.

We value dignity, respect and inclusion and offer a supportive team and regular monthly RDO.

Qualifications

• Experience in a support coordination or similar role (essential).
• Degree in Allied Health or a Cert 3/4 in Disability, Youth Work, Welfare or Community Services.
• Strong written and verbal communication with attention to detail.

Responsibilities

• Assist existing and new clients to implement NDIS plans and coordinate supports.
• Build and maintain relationships with service providers and disability groups in the region.
• Promote Focused on Care's support coordination services and grow client base.
