Job summary

Tweed Coast Support Services is seeking an experienced NDIS Support Coordinator to join one day a week, with potential for growth. The role focuses on helping participants maximise their NDIS plans, connecting with providers and allied health professionals, and monitoring progress across the Gold Coast and Northern Rivers.

You will coordinate supports, help participants build capability to manage their plans, and ensure clear understanding of how funding can be used.

Qualifications

• Demonstrated support coordination experience
• Experience with people with psychosocial, neurological and neurodivergent diagnoses
• NDIS Worker Screening Check (or willing to obtain before starting)
• Completion of NDIS mandatory training (or willing to complete)
• A valid driver's licence and reliable, insured vehicle
• Preferred: qualifications in disability services, social work or related field

Responsibilities

• Coordinating supports and helping participants access services
• Get supports in NDIS plan up and running
• Build participants' capacity to coordinate their supports
• Ensure participants understand their plan and how to use it
• Build relationships with providers and allied health
• Review plans and track progress against goals
• Keep accurate records and report to NDIS on time
