Country & Outback Health is seeking an NDIS Support Coordinator to maximise participants' plans, build independence and progress towards their goals. You will link participants with services and provide high-quality, person-centred support, with potential CPSP involvement to identify and progress recovery goals and connect clients with local services.

The role requires resilience, strong organisation, and the ability to work both independently and within a team; experience with MS Office and CRM

Qualifications

• Experience in NDIS Support Coordination preferred.
• Experience in disability, community or mental health sector.
• Compassionate, resilient and a natural problem solver.
• Confidence working independently while contributing to a supportive team.
• Strong organisational and time-management skills and ability to manage a caseload independently.
• Excellent communication skills and ability to build relationships.
• Proficient in MS Office and CRM platforms.
