Job summary

Tender Loving Care Disability Services – Bankstown NSW is seeking a Support Coordinator to coordinate support services for clients, assist them in achieving their goals, and promote staff and volunteers in a learning, values-driven environment.

This role requires engaging with service providers and allied health professionals, monitoring progress toward plan goals, and guiding clients to access community supports. A minimum of 2 years in disability or related services is preferred.

Qualifications

• Minimum 2 years of experience in disability or related community services field.
• Proven experience in high-level Support Coordination under the NDIS.
• Deep understanding of NDIS processes, price guide, service delivery expectations and relevant legislation.
• Strong commitment to person-centred, trauma-informed and culturally responsive practices.
• Excellent interpersonal, advocacy and conflict resolution skills.
• Strong administrative and organisational capabilities including CRM proficiency.

Responsibilities

• Coordinate and manage support services for clients to help achieve their goals.
• Engage with service providers and allied health professionals to coordinate care.
• Monitor progress toward plan goals and adjust supports as needed.
• Assist clients to identify community or mainstream supports.
• Establish and maintain links with NDIS, TLC programs and other provider services.

The Support Coordinator will play a vital role in our organization, ensuring seamless coordination of support services for our clients and assisting them in achieving their goals and aspirations. The Support Coordinator will promote a learning focused, valuing and cooperative organization that promotes the development and wellbeing of staff and volunteers.

This role will be responsible in engaging with appropriate service providers and allied health professionals, and monitor progress toward the achievement of their plan goals. The Support Coordinator is to assist customers to identify community or mainstream supports that can also assist them to achieve their set plan goals.

Someone who has experience in coordinating and assisting in the management of a caseload of complex and potentially at-risk clients.

Someone who can develop and support programs that promote skill development and encourage contribution in leisure, sport, recreational activities in the community. Promote and support clients to access connections and networking opportunities outside of the service system, aligned with individual goal;

Someone who can assist customers to build their capacity around the understanding of their plan and implementation;

Someone who can establish and maintain effective links with the NDIS, TLC programs and other provider services.
