Qualifications

• NDIS Support Coordination, disability, community services, mental health, or case management experience.
• Strong communication and relationship-building skills.
• Ability to manage workloads independently and prioritise caseloads.

Responsibilities

• Support participants to understand and implement their NDIS plans.
• Identify providers and coordinate services across multiple providers.
• Maintain accurate records and reports.
