Job summary

Community Solutions Group is seeking an NDIS Support Coordinator in Toowoomba to help participants achieve greater independence by assessing needs and arranging supports. You will work closely with participants to identify services, coordinate plans and address barriers with action plans, aiming for optimal outcomes.

You will build strong relationships with participants, families and providers; travel to visit participants; manage billable hours, documentation and IT systems to ensure accurate

Qualifications

• Thorough understanding of the NDIS and NDIS plans/budgets.
• Experience with billing targets and case noting.
• High level administration experience and IT skills (Excel).
• People person with disability sector experience and ability to motivate participants and families.
• Business mindset with customer service focus.
• Excellent time management and organisation skills.

Responsibilities

• Assess participants' needs and identify supports to reach goals.
• Coordinate supports and maximize NDIs plans for independence.
• Visit participants as required and build rapport with families and providers.
• Negotiate with providers and explain cost allocations within NDIS budgets.
• Ensure service agreements are completed and maintain records.
• Manage billing targets and accurate case notes.
