Job summary

Just Better Care Townsville and Cairns is seeking an experienced NDIS Support Coordinator to assist participants with implementing their NDIS packages. You will coordinate access to disability, community, health, housing and education services, keep budgets on track, and support participants toward their goals.

The role is full-time and based in Townsville, requiring strong communication, organisation and stakeholder engagement with participants, families, providers and community organisations

Qualifications

• Existing, adequate experience as a Support Coordinator.
• Certificate III/IV or Diploma in Disability, Community Services, Individual Support, Mental Health or related fields, or equivalent industry experience.
• Extensive understanding of the National Disability Insurance Scheme.
• Experience working with people with a disability, families and community support providers.
• Strong communication, organisation, problem-solving and time management skills.
• Competence with Microsoft Outlook, Excel and client management systems.
• Current Working with Children Check and NDIS Worker Screening Check.

Responsibilities

• Work with participants to understand their NDIS plan and goals.
• Coordinate access to disability, community, health, housing, education and mainstream services.
• Assist participants and families to understand and use funded supports effectively.
• Build and maintain positive relationships with participants, carers, providers and external stakeholders.
• Support service bookings, referrals, appointments and follow-up actions.
• Maintain accurate client notes, records and service documentation.
• Promote choice and control, independence, inclusion and safe service delivery.
• Respond to enquiries, incidents or concerns in a timely and professional manner.
• Contribute to continuous improvement, compliance and quality service practices.
