Job summary

Community Solutions is seeking an NDIS Support Coordinator in Toowoomba to help participants achieve independence by coordinating supports and maximizing their NDIS plans. You will assess needs, identify services, and implement action plans with strong relationships to participants, families, and providers.

Responsibilities include coordinating supports, negotiating with providers, and ensuring service agreements, with a focus on capacity building and overcoming barriers.

Qualifications

• Experience with NDIS and understanding of NDIS Plans/Budgets.
• Experience with billing targets and case noting.
• High level administration experience and IT skills, particularly Excel.

Responsibilities

• Build strong relationships with participants, families, and external providers.
• Drive to visit participants as needed.
• Help participants negotiate with providers on services and costs within NDIS budgets.
• Work with billable hours and targets.
• Ensure service agreements are completed.
• Take a holistic approach to capacity building and address barriers for participants.
