Job summary

Compassionate Community Care Pty Ltd seeks an experienced NDIS Support Coordinator to implement participants' plans, maximize funding value and build capacity to navigate supports. The role includes home visits, coordination with NDIA and allied health professionals, and timely reporting.

Part-time and flexible working arrangements are available, with a diverse participant caseload and ongoing professional development opportunities within CCCare.

Qualifications

• NDIS Level 2 Support Coordination knowledge and experience.
• Strong written communication and report writing skills.
• Excellent stakeholder engagement and relationship-building abilities.
• Ability to work independently within a supportive team.
• Understanding of safeguarding, privacy, and professional boundaries.

Responsibilities

• Implementing participants' NDIS Plans and supporting them to achieve their goals.
• Helping participants understand their NDIS funding and maximise supports.
• Building participant capacity to navigate the NDIS and coordinate supports.
• Managing an appropriate caseload with agreed working hours.
• Connecting participants with formal, informal, community and mainstream supports.
• Maintaining relationships with participants, families, providers and stakeholders.
• Communicating with allied health professionals, NDIA representatives and LACs.
• Conducting home visits and meetings with participants and stakeholders.
• Preparing implementation, progress and reassessment reports within required timeframes.
• Maintaining accurate case notes and records of delivered supports.
