Accept Care in Adelaide is seeking an experienced NDIS Relationship Partner to build long-term relationships with participants, coordinate supports, and develop referral networks. This role involves onboarding new participants, coordinating services, and ensuring responsive, high-quality care in partnership with our support workers and team.

Based from the North Adelaide office with up to two days of work-from-home, the position requires travel across metropolitan Adelaide and a vehicle with

Qualifications

Responsabilités

• Building strong relationships with participants, families and their support networks
• Developing referral relationships and connections across the local NDIS community
• Managing new participant enquiries, onboarding and the commencement of services
• Coordinating and reviewing supports as participants’ needs and goals change
• Working closely with Support Workers and the wider team to deliver great participant experiences
• Maintaining regular contact with participants and responding to day-to-day service needs
• Supporting NDIS compliance, quality and safe service delivery
