Job summary

Accept Care is seeking an experienced NDIS Relationship Partner to coordinate supports and build long-term relationships with participants across Adelaide. You’ll manage the participant journey from enquiry to ongoing service coordination, working with our team to deliver reliable, person-centred care.

This role offers a hybrid working model with flexibility for up to two days per week remote and regular travel across metropolitan Adelaide to meet participants in their homes.

Qualifications

• Experience in NDIS or disability services is essential.
• Proven ability to build and maintain relationships with participants, families and stakeholders.
• Strong communication and organisational skills.

Responsibilities

• Build strong relationships with participants, families and support networks.
• Develop referral relationships within the local NDIS community.
• Manage new participant enquiries, onboarding and start of services.
• Coordinate and review supports as participants' needs change.
• Collaborate with Support Workers and delivery teams to ensure quality experiences.
• Maintain regular contact with participants and respond to day-to-day needs.
• Support NDIS compliance, quality and safe service delivery.
